The following forms should be attached to all software purchase orders that are submitted. Please choose the appropriate form that meets your needs.
Please note: You must be logged into your Alief Google account to access the forms listed below.
Alief Software Approval Form
Use this form to request software that has never been used before in Alief I.S.D.
Print the form and fill it out completely.

Use this form to request software that has been used before, whether on your campus or another Alief I.S.D. campus.

Alief iPad App Evaluation Request Form
Use this form to request an iPad app that is not currently on the Approved Digital Tools List.
Check the Approved Digital Tools List before you complete the application form.
Print the form and fill it out completely.
Complete the form with the assistance of your campus Instructional Technology Specialist or Library Information Specialist.
Completed forms should be submitted to the designated Technology Integrator for your campus.
